Help empower individuals at SLCS as a Case Worker in our Supported Independent Living (SIL) Program. Engage with adults experiencing disabilities, providing essential support to enhance their independent living.
Recognized for its commitment to community support, SLCS is hiring Full time Case Workers. As part of our interdisciplinary team, you will create individual case plans, educate clients on life skills, and connect them to necessary resources. This position may require handling clients who display verbal or physical aggression, utilizing de-escalation techniques as needed.
Key Responsibilities:
• Create and implement individual case plans for clients • Facilitate connections to community supports for goal achievement • Provide life skills education to promote independence • Assess risk levels and safety needs of clients • Document client interactions and case notes thoroughly
Requirements: • Post-secondary education from an accredited institution • 1-3 years of relevant experience in related fields • Experience using de-escalation strategies effectively • Valid G Class Driver's License with reliable vehicle • CPR/First Aid Level C certification
