leisure lifestyle and access, employment, and education are addressed in this planEnsures the client’s ongoing legal, mental, and physical health issues are addressed in the planWhen and as needed, provides a wide range of interventions to enable the client to address and accomplish the established goalsInteracts with other professionals, family members, and organizations to ensure services, admissions, and treatments are in the client’s interest and supportive of the long-term goalsOn the client’s behalf, is a resource to other professionals, including but not limited to the courts, probation services, and community and institutional psychiatric servicesMaintains extensive records and case notes in accord with Community Living British Columbia policies and standardsCompletes monthly progress reportsParticipates in staff and resident/staff meetings as instructed by the Program ManagerOther duties as subsequently negotiatedEXPERIENCE AND SKILLSDiploma or Certificate in Social Services or a relevant field with one year of related experienceCounselling experience in the appropriate field and demonstrate a clear ability to respond well in crisis situationsExperience with clients with a history of conflict with the law, developmental disabilities and/or spectrum disorders, interaction with social agencies and experiencing psychiatric/personality disordersExperienced with working with individuals with active substance abuse challenges and willingness to apply harm reduction approachesUnderstanding of providing service to people with different learning abilitiesExtensive knowledge of community services in Lower Mainland and Fraser Valley as they relate to persons with psychiatric involvementExperience in assessment,
