Under the guidance of the Social Work Site Leader, this role requires a Master’s degree in Social Work and strong communication skills to effectively support the interdisciplinary team. You will conduct assessments, facilitate care planning, and provide education to staff to enhance patient outcomes.
Key Responsibilities:
• Perform thorough psychosocial assessments and patient interviews • Maintain up-to-date patient/resident documentation and records • Collaborate in interdisciplinary teams for coordinated patient care • Deliver educational presentations and training sessions to staff • Participate in program evaluation and research initiatives
Requirements: • Master’s degree in Social Work and registration with BC College • At least one year of experience in a similar role • Proven ability to communicate with diverse healthcare professionals • Capacity to manage conflict and lead group discussions • Enthusiastic about teaching and supervising social work students
