The Transitional Housing Case Worker will support participants of all backgrounds who are living in our transitional housing for women, gender diverse folks, and children who are exiting situations of violence. They will support people on their healing journey, through one-on-one support, case coordination, program delivery, system navigation, advocacy and referrals to relevant resources.ResponsibilitiesTo provide individual support and ongoing case management to people who have been impacted by violence to set goals and create plans to reach those goalsTo empower people to advocate for themselves and to advocate on their behalfTo assist people with finding permanent housing and connecting them to income supportsTo provide information, referrals and system navigation to participants and their familiesRegular follow‑ups and checks‑ins with transitional housing tenants, including wellness checksSupport participants to prepare for external appointments and accompany participants to offer support and companionship as neededTo work in collaboration with internal and external programs to ensure effective wrap‑around supports for participantsMaintain positive working relationships with service providers, and advocate to them on behalf of participants as neededReport on program outcomes to the Director of Community Development as required by funding agreementsEssential Knowledge,
