Vancouver Coastal Health (VCH) invites applications for a Community Health Nurse on the Home Health Team at Pacific Spirit Community Health Centre in Vancouver, BC. The nurse will work within an interdisciplinary team to assess, plan, implement, and evaluate nursing care for clients with acute, chronic, and palliative needs. The role includes care coordination, client/family education, and transitioning services across community settings, guided by BC standards and VCH values.
