Ontario Health atHome is seeking an experienced Care Coordinator (Full-Time) in Timmins to assess patient care needs, determine eligibility and coordinate with patients, families and providers for connected, patient-centred care. You will link patients to community services, monitor care plans, and support a busy multidisciplinary team.
Must hold an Ontario regulatory license (RN/Physiotherapist/OT/Social Worker), have community health experience, and solid communication and time-management
