As part of this permanent full time position, you'll engage with various health and social care professionals to conduct frailty assessments and develop effective care plans. Your dedication to delivering high-quality, person-centred care will play a significant role in helping patients remain in their homes and avoid hospital admissions.

Key Responsibilities:
Perform detailed assessments for older adults facing frailty
Aid in clinical decision-making and management guidance
Enable safe discharges and admission avoidance




Collaborate with multidisciplinary teams in health and social care
Provide leadership and expert advice as needed

Requirements:
Valid NMC UK pin and healthcare qualified status
Experience in urgent care or community services
Exceptional clinical assessment and communication skills
Proven leadership capabilities within a healthcare setting
Postgraduate qualifications in advanced practice preferred
