Position Summary The Nurse Practitioner (NP) provides comprehensive, evidence-based, patient-centered care to patients across a broad range of health conditions. The NP conducts clinical assessments, diagnoses and manages acute and chronic conditions, develops individualized treatment plans, and provides medication management and ongoing follow-up care. The role is primarily delivered through virtual/telehealth services, with opportunities to support patients across Canadian provinces and territories, subject to applicable licensure and regulatory requirements.
The NP works collaboratively with physicians, pharmacists, and other healthcare professionals to promote positive patient outcomes, ensure continuity of care, and deliver accessible, high-quality healthcare.
Key Responsibilities
Clinical Assessment &
- Diagnosis
- Conduct comprehensive health assessments for adolescents and adults through virtual care.
- Obtain detailed medical, surgical, medication, family, and social histories.
- Assess, diagnose, and manage a broad range of acute and chronic health conditions.
- Review patient records, laboratory results, diagnostic reports, and other relevant clinical information.
- Identify potential underlying medical conditions and determine appropriate investigations.
- Develop and document clinical assessments, diagnostic impressions, and treatment recommendations.
- Recognize patients requiring urgent or specialized care and facilitate appropriate referrals.
Treatment &
- Medication Management• Develop individualized, evidence-based treatment plans in collaboration with patients.
- Initiate, prescribe, adjust, and monitor medications in accordance with clinical guidelines, applicable regulations, and provincial scope of practice.
- Monitor treatment effectiveness, medication adherence, side effects, and patient outcomes.
- Order and interpret appropriate laboratory tests and diagnostic investigations when clinically indicated.
- Provide non-pharmacological treatment recommendations and preventive health guidance.
- Monitor patient progress and modify treatment plans based on clinical response and changing needs.
- Provide ongoing follow-up and management through virtual/telehealth appointments.
Patient Education &
- Support• Educate patients and families about health conditions, treatment options, medications, and available healthcare resources.
- Provide guidance on lifestyle, preventive care, health promotion, and self-management.
- Support patients in understanding their treatment plans and making informed decisions about their care.
- Address patient questions, concerns, and barriers to care in a timely and professional manner.
- Build strong therapeutic relationships and patient trust within a virtual care environment.
- Promote continuity of care through appropriate follow-up and patient engagement.
Collaboration &
- Care Coordination• Collaborate with physicians, pharmacists, specialists, and other healthcare professionals.
- Coordinate referrals to specialists, diagnostic services, community resources, and other appropriate providers.
- Facilitate continuity of care between primary care, virtual care, and other healthcare services.
- Participate in multidisciplinary case discussions, clinical consultations, and quality improvement initiatives.
- Support appropriate escalation of patients whose clinical needs exceed the scope of virtual or primary care.
Documentation &
- Compliance• Maintain accurate, timely, and comprehensive clinical documentation within the electronic medical record (EMR).
- Ensure compliance with provincial nursing standards, regulatory requirements, privacy legislation, and organizational policies.
- Maintain appropriate documentation related to assessments, diagnoses, treatment plans, prescriptions, referrals, and follow-up care.
- Participate in clinical audits, quality assurance activities, and continuous professional development.
- Stay current with evolving clinical guidelines, evidence-based practices, and regulatory requirements.
Qualifications
- Current registration as a Nurse Practitioner in good standing with the applicable provincial nursing regulatory body.
- Authorized to practice as an NP in any Canadian province or territory, depending on assigned patient population and service requirements.
- Authorized to assess, diagnose, prescribe, and manage patients within applicable provincial scope of practice.
- Minimum 2–3 years of Nurse Practitioner experience preferred.
- Previous experience providing telehealth or virtual healthcare services strongly preferred.
- Experience in primary care, family practice, community health, or another relevant clinical setting strongly preferred.
- Demonstrated experience in comprehensive patient assessment, diagnosis, treatment planning, and ongoing management.
- Experience with medication management and preventive healthcare.
- Comfortable using EMRs, telehealth platforms, and digital healthcare tools to support patient care and clinical documentation.

- Solid organizational skills and attention to detail.
