Elevate mental health services at the Centre for Family Medicine as a Registered Social Worker or Psychotherapist in KW4. This position supports patient access to mental health care.

In this 0.6 FTE role, you will work with a multidisciplinary team to provide comprehensive care and counseling. Your work with physicians and community organizations aims to enhance support for patients facing mental health challenges, including leading mental health groups as needed.

Key Responsibilities:
• Build qualified relationships with healthcare partners
• Perform assessments and manage diverse mental health issues
• Facilitate mental health workshops for community groups
• Communicate patient needs effectively with providers
• Document case notes in EMR systems

Requirements:
• Master's degree in Social Work or Psychotherapy
• Registered with relevant College and holds liability insurance
• Minimum of 3 years clinical experience preferred
• Strong interpersonal and communication skills
• Experience with marginalized and at-risk populations
