As a member of VCH's interdisciplinary team, you will assess patient needs and provide counseling to help reduce reliance on healthcare services. You will navigate crisis situations, collaborate with police, and ensure continuity of care for vulnerable populations. Key Responsibilities:
• Conduct mental and physical assessments of clients
• Collaborate with police for effective outreach
• Develop and adjust integrated care plans
• Facilitate referrals to community services
• Participate in case conferences for client progress
Requirements:
• Registered Nurse or Psychiatric Nurse in BC
• Two years in mental health/substance use care
• Valid BC Drivers License and vehicle access
• Knowledge of crisis intervention techniques
• Ability to work independently and in teams
