As a Social Worker, your work will focus on assisting various patient populations, including those in acute medical and geriatric care. You'll conduct assessments, manage crises, and oversee discharge planning, all while ensuring a smooth patient flow within the healthcare system. Your skills in liaising with community resources will enhance the support offered to patients and their families.

Key Responsibilities:
• Provide comprehensive psychosocial assessments across different units
• Collaborate on discharge planning initiatives with healthcare teams




• Assist patients and families in accessing community resources
• Flexible management of caseloads in high-pressure situations
• Supervise social work students during their practicum

Requirements:
• MSW from an accredited Social Work program required
• Current or eligible membership with OASW and OCCSWSSW
• Experience in discharge planning and acute care settings preferred
• Proficient in electronic health documentation
• Solid interpersonal and decision-making skills
