Job Summary

Responsibilities
- Deliver comprehensive patient care by assessing health conditions, administering medications, and implementing treatment plans in accordance with physician orders.
- Utilize electronic health records systems such as Epic and Cerner to document patient information accurately and efficiently.
- Collaborate with multidisciplinary teams to coordinate case management, discharge planning, and follow-up care.
- Perform clinical procedures including phlebotomy, wound care, pain management interventions, and dementia or senior care strategies.
- Monitor vital signs, manage acute care needs, and respond swiftly to emergencies with professionalism and confidence.
- Educate patients and their families on medical terminology, medication administration,
and self-care techniques to promote health literacy.
- Maintain compliance with healthcare regulations and standards while demonstrating excellent patient care practices in hospital, assisted living, or home care environments.
Experience
- Proven experience working in hospital settings with a strong background in acute care and patient management.
- Familiarity with electronic health record systems such as Epic and Cerner is highly preferred.
- Prior experience in pediatrics, senior care—including dementia or assisted living environments—is advantageous.
- Demonstrated ability to handle laboratory procedures like phlebotomy and manage medication administration confidently.
- Knowledge of medical terminology combined with hands-on experience in pain management and case management roles.
- Previous roles involving home health or nursing home settings are beneficial for understanding diverse patient needs.
- Strong communication skills with experience working with children or elderly populations to ensure compassionate, effective care.

Pay: $33.08-$40.00 per hour
