Join Island Crisis Care Society in enhancing community outreach efforts for at-risk individuals. As a Community Support Worker, you will engage with those facing homelessness, promoting stability and well-being.
In this role, you will identify and connect with individuals in need, assessing their circumstances and developing impactful support plans. Applying compassionate communication techniques, you will provide guidance and support for clients as they tackle housing-related issues. Engaging with an integrated case management team enhances collaboration and improves client outcomes.
Key Responsibilities:
• Identify individuals at risk and build supportive relationships
• Gather insights on client needs through various assessment methods
• Create short-term intervention plans based on client feedback
• Facilitate emotional support and problem-solving techniques
• Collaborate with service providers and document client progress
Requirements:
• Degree in Social Services or equivalent qualification
• Two years of relevant experience in a similar role
• Familiarity with mental health and addiction support practices
• Solid interpersonal and collaboration abilities
• Commitment to ethical practice and self-care
