This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Senior, Care Management based in Canada.

This is a fully remote senior care management opportunity focused on coordinating complex patient cases and supporting continuity of care.

You’ll help patients navigate clinical, social, and access-related barriers throughout their treatment journey.

The role involves close collaboration with healthcare providers, pharmacies, payers, and internal teams.

You’ll assess cases, support care plans, facilitate transitions, and help resolve issues that may affect patient outcomes.

You’ll also analyze recurring case and workflow challenges and contribute to practical process improvements.

As a senior team member, you’ll provide guidance on complex cases, escalations, documentation, and best practices.

Accountabilities

- Plan and manage complex care coordination cases to support timely patient access, treatment progression, and continuity of care.
- Conduct patient and case assessments, contribute to care planning, and facilitate transitions of care while addressing clinical, social, and access barriers.
- Coordinate with healthcare providers, pharmacies, payers, patients, and internal partners to resolve access challenges and maintain continuity throughout each case.
- Identify recurring case issues, workflow patterns, and root causes, using findings to recommend process improvements and enhance operational effectiveness.
- Develop and refine care coordination tools, job aids, templates, and documentation standards to promote consistency and reduce rework.
- Guide and support less experienced team members with complex cases, escalation procedures, documentation standards, and effective case-management practices.
- Communicate clearly and professionally with patients, clients, healthcare professionals, and cross-functional teams, including English-speaking stakeholders.




- Manage sensitive situations and differing perspectives with professionalism, sound judgment, and appropriate negotiation skills.
- Maintain accurate documentation, meet established deadlines, and manage multiple priorities in a fast-paced setting.
- Participate in approximately 15% annual travel as required by the role.

Requirements

- Bachelor’s degree, registered nursing qualification, graduate-level education, or equivalent relevant experience; a healthcare-related educational background is preferred.
- Approximately 5 years of nursing or relevant clinical experience, with at least 2 years of experience in care management, clinical support, pharmaceutical, insurance, public/private reimbursement, or a related environment preferred.
- Experience in healthcare and case management, medical practices, hospitals, insurance, or biopharmaceutical settings is an asset.
- French and English bilingualism is preferred.
- Strong patient-focused mindset with excellent customer service and interpersonal skills.
- Excellent verbal and written communication skills, including the ability to understand and communicate medical information effectively.
- Strong analytical, mathematical, research, and problem-solving abilities, with the capacity to identify causes, evaluate information, and develop effective solutions.
- Strong organizational skills and attention to detail, with the ability to multitask, prioritize competing demands, and consistently meet deadlines.
- Ability to work independently while contributing effectively within a collaborative, cross-functional team.
- Demonstrated ability to work under pressure and handle sensitive situations, differing opinions, and complex discussions professionally.
- Solid leadership, presentation, relationship-building, and negotiation skills.
- Strong computer proficiency,



including Microsoft Word, Excel, PowerPoint, and other Office applications.
- Strong business and financial awareness, combined with sound judgment and a practical approach to problem solving.
