Secure your role in the Mental Health Services department, where you will report to the Manager and deliver essential care to older adults struggling with mental health and neurological conditions. Your work will include assessments, counselling, and collaboration with families and healthcare teams to support patient well-being.

Key Responsibilities:
• Assess patient mental status and psycho/social needs
• Provide compassionate counselling and support to families
• Engage in interdisciplinary teamwork for comprehensive care




• Document patient progress and collaborate on treatment plans

Requirements:
• Current registration with the College of Nurses of Ontario
• At least two years of geriatric assessment experience
