Elevate patient care as a Social Worker in the Nephrology Program at Humber River Health. This permanent, full-time role involves providing psychosocial support and coordinating care for patients living with chronic kidney disease.
As a Social Worker, you'll play an essential role in managing patient care by conducting assessments, offering counseling, and collaborating with a diverse healthcare team. You'll also engage community resources to support patient discharge and advocate for their needs throughout the treatment process.
Key Responsibilities:
• Provide psychosocial assessments and counseling
• Plan and coordinate discharge for nephrology patients
• Serve as a resource to interprofessional team members
• Develop partnerships with community support services
• Manage patient caseload effectively in an acute setting
Requirements:
• MSW from an accredited institution needed
• Membership with OASW and OCCSWSSW required
• Relevant nephrology and dialysis experience preferred
• Robust organizational and communication skills
• Capability to work independently and collaboratively
