Clinical Community Pharmacist

Twin Bridges Pharmacy & Infusion Clinic | Sarnia, Ontario

Compensation: $70–$75/hour

Schedule: Full time, 40 hours/week, Monday–Friday only

Vacation: 3 weeks

Twin Bridges Pharmacy & Infusion Clinic is looking for a pharmacist who wants the prospect to practice differently.

We are building a new community pharmacy inside the Twin Bridges Nurse Practitioner-Led Clinic in Sarnia, designed around pharmacist-led patient care, specialty pharmacy, infusion services and close collaboration with other healthcare professionals.

This is not intended to be a traditional high-volume dispensing position. Our pharmacist-at-intake workflow is designed so pharmacists spend their time with patients, making clinical decisions and collaborating with onsite Nurse Practitioners and other healthcare professionals, while other team members handle work that does not require a pharmacist.

You do not need to arrive as a specialty pharmacy expert or experienced pharmacy manager. You do need to be clinically curious, committed to excellent patient care, and excited about helping us build something new.

The Role

As a pharmacist at Twin Bridges, you will:

- Practice at the full scope of your license, including medication management, minor ailments, vaccination, medication reviews and follow-up care.
- Work closely with onsite Nurse Practitioners, including use of the clinic EMR and collaborative patient-care planning.
- Identify drug therapy problems and take an active role in resolving them.
- Develop expertise in specialty pharmacy and infusion therapy, including therapy coordination,



authorization processes and patient support.
- Build collaborative relationships with specialists, Bluewater Health, primary care providers and other regional healthcare partners.
- Help develop new pharmacist-led clinical services, patient-care pathways and future scope expansions.
- Help shape pharmacy workflow, policies and operations as the practice grows.
- If interested, develop your skills in pharmacy leadership and business management, with a pathway toward becoming Designated Manager and potentially participating in the long-term ownership of the pharmacy.

Who We're Looking For There isn't one specific pharmacy background we're looking for.

You may be a community pharmacist who wants more opportunity to practice clinically, a hospital or ambulatory-care pharmacist interested in a different model of community practice, or someone earlier in your career looking for a place to develop those skills.

What matters most is that you are a thoughtful pharmacist who enjoys patient care, works well with other healthcare professionals and wants to practice at the full scope of the profession.

Required:

- Licensed and in good standing with the Ontario College of Pharmacists, or eligible for licensure in Ontario
- Strong clinical judgment and commitment to patient-centred care
- Strong communication and collaborative skills
- Injection and immunization certification, or willingness to obtain it

Experience in community, hospital,



specialty or ambulatory pharmacy, infusion therapy, complex medication management, leadership or business development are all assets, but none are prerequisites. We are willing to invest in developing the right person. Compensation & Growth The position offers:

- $70–$75/hour
- 40 hours/week, Monday–Friday
- No weekends or holidays

The successful candidate will initially join as a Staff Pharmacist. For someone interested in leadership, there is a pathway toward becoming the pharmacy's OCP Designated Manager, potentially within approximately six months based on mutual fit and readiness. Longer term, we want the pharmacist who helps lead and grow Twin Bridges to participate meaningfully in its success. This may include profit sharing and, for the right long-term candidate, an equity/ownership opportunity.
