We’re hiring! A dynamic and well-established primary care team in Eastern Ontario is seeking a Full-Time Nurse Practitioner to join their team-oriented and patient-centered environment.
As a key member of an inter-professional healthcare team, the Nurse Practitioner will work closely with physicians and allied health professionals to provide comprehensive primary care services. This role involves engaging with patients to manage their personalized health needs, including health promotion, chronic disease management, and preventative care—working to the full scope of the Nurse Practitioner role as defined by the College of Nurses of Ontario.
What You’ll Do:
- Provide primary care services in a collaborative team environment
- Conduct comprehensive health assessments, diagnosis, treatment planning, and follow-up care
- Prescribe medications, including controlled substances, in accordance with CNO standards
- Participate in quality improvement and inter-skilled case reviews
- Promote wellness and preventive health strategies for individuals and families
What We’re Looking For:
- Current registration as a Registered Nurse in the Extended Class with the College of Nurses of Ontario (CNO)
- Completion of a recognized Primary Health Care Nurse Practitioner program (NP-PHC)
- Master’s degree in Nursing or a related graduate degree with a clinical focus (preferred)
- Completion of an accredited controlled substances course
- 3–5 years of Nurse Practitioner experience (Primary Health Care experience preferred)
- Strong clinical decision-making and communication skills
