Pediatric Hematology and Oncology within Acute Children's Services encompasses an inpatient and outpatient component. This includes acute tertiary and subspecialty oncology medical and surgical care for children ages 0-18 years from the Central-West region within a family-centered environment. The pediatric neuro-oncology clinic and pediatric neuro-oncology aftercare clinic work within the hematology and oncology pediatric program.

The program provides care of children from hospital to home in all settings in the Children's Hospital including 3B2, an inpatient hematology oncology unit; and 3F, an ambulatory medical day unit with a full spectrum of pediatric hematology oncology clinics.

Through in-depth psychosocial assessments, the Social Worker will identify child and family strengths, problem-solving abilities, dynamics, interactions, attitudes, feelings and cultural issues in relation to the child’s illness.

The Social

Worker will develop and implement a plan of intervention, which addresses coping, adaptation, compliance and quality of life. A variety of modalities including counseling, group work, consultation, case management, community outreach and advocacy will be employed.

Worker will work collaboratively with the health care team and identify and make referrals to appropriate supports and resources.

Worker will integrate research and practice.

- Master’s Degree in Clinical Social Work from an accredited School of Social Work

- Current Certificate of Registration with the Ontario College of Social Workers and Social Service Workers

- Minimum three (3) years clinical experience (within the last 6 years) with children and families dealing with an oncology diagnosis, dealing with death and dying, bereavement issues and chronic progressive illness

- Demonstrated expertise in assessment, counseling, crisis intervention, advocacy and teaching

- Excellent communication, teamwork skills, and the ability to collaborate effectively within an interdisciplinary team

- Demonstrated ability to work efficiently in a fast paced changing environment

- Proven ability to organize, prioritize, and problem solve effectively

- Experience and demonstrated familiarity with Child & Family Services Act, Consent to Treatment Act, and Mental Health Cat, and other related legislation specific to the role

- Expert knowledge about community resources, referral processes, and understanding of research for the pediatric hematology and oncology population

- Ability to creatively problem solve and facilitate change

- Demonstrated understanding of the social determinants of health with dedication to equity, diversity and inclusion

- Demonstrated proficiency in report writing relating to assessment, and consultation, and strength in case management and consultation

- Excellent written and verbal communication alongside strong interpersonal skills

- Evidence of self-motivation, enthusiasm, and flexibility

- Demonstrated leadership and delegation abilities and the ability to organize and coordinate patient and family-centered care with other members of the healthcare team

- Qualified as a field instructor for social work students and demonstrated experience supervising students

- Demonstrated ongoing professional development and commitment to corporate initiatives
