Support residents at Park Place Home as a part-time Social Worker on a flexible schedule. Your focus will be on improving resident transitions and overall care. This part time independent contractor position offers 22.5 hours weekly, centered on assessing and coordinating care for residents during their move-ins and move-outs. You will also assist families in accessing necessary resources, ensuring residents have the support they need for a seamless transition. Key Responsibilities:

- Evaluate and assist residents in transitions
- Document assessments and provide follow-up support
- Collaborate with interdisciplinary teams
- Facilitate Memorial Services with Recreation Department
- Engage in quality improvement initiatives Requirements:
- Bachelor’s degree in Social Work
