Job Summary

We are hiring a full-time, temporary Social Work Intensive Case Manager (ICM). This job will run until November 13, 2027.

The Social Work ICM is part of a team providing services to complex clients with a history of homelessness in a community based setting. Clients may have mental health concerns, substance use, physical health concerns, and/or cognitive concerns. The ICM is responsible for providing front-line recovery-orientated services.

The ICM works under the direct supervision of the Wahkohtowin Team Leader, and and under the overall direction of the Manager and Director of Operations. The ICM is responsible for front line responsibilities and for expertise, knowledge and skills related to overall service delivery to residents including overseeing Case Management related issues of their caseload of 12 individuals.





The ICM is responsible for providing recovery-orientated services that will assist residents and their care team in developing holistic wellness plans. Applicants must provide your current registration &/or be eligible for professional registration.

Hours of work are Monday-Friday day shifts, with on call responsibilities as needed.

Pay: $32.02-$33.30 per hour

Education:
- Bachelor's Degree (required)

Licence/Certification:
- Saskatchewan Class 5 Driver's License (required)
- First Aid Certification (preferred)
