Support residents at Park Place Home as a part time Social Worker on a flexible schedule. Your focus will be on improving resident transitions and overall care.

This part-time independent contractor position offers 22.5 hours weekly, centered on assessing and coordinating care for residents during their move-ins and move-outs. You will also assist families in accessing necessary resources, ensuring residents have the support they need for a seamless transition.

Key Responsibilities:

- Evaluate and assist residents in transitions
- Document assessments and provide follow-up support
- Collaborate with interdisciplinary teams
- Facilitate Memorial Services with Recreation Department
- Engage in quality improvement initiatives

Requirements:

- Bachelor's degree in Social Work
- Current registration with BC Association of Social Workers
- Ability to work independently and communicate effectively
- Interest in enhancing resident quality of life
- Experience in residential care is beneficial
