Case management is provided to patients receiving services from Home Care, long term care and residents in personal care homes. Core functions include referral/intake, engagement, assessment, care planning, monitoring, evaluation, termination/discharge in collaboration with family, care givers, and multi-disciplinary team members.
This position liaises with other area and provincial services. The position reports to a manager and a senior position(s) for daily clinical supervision. This position collaborates with acute care, rehabilitation, home care, long term care, mental health, government and non-government agencies to assess and coordinate services to meet needs.
