The Indigenous Registered
Nurse - Case Manager is responsible for providing advances patients liaison services, including discharge, follow-up, and transitional planning. In your role as a Registered Nurse (RN), you will provide support for children and their families during their hospital stay and support their transition from Stollery to the community. You will be working within a patient family centered approach and will be responsible for assessing, planning, implementing and evaluating patient care in conjunction with the interdisciplinary and community teams. The RN will collaborate with Awasisak team members, HCP's and professionals from different disciplines to better serve the Indigenous patients and families.
Description
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS.
PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification.
Additional Required Qualifications
As a bona fide occupational requirement and under Section 11 of the Alberta Human Rights Act, only Indigenous applicants will be considered. A high degree of cultural competence embedded experience as it related to working with Indigenous people. At least 5 years' experience providing significant direct health care services to Indigenous families/communities or being someone who has live at minimum 5 consecutive years as a participant member within Indigenous community.
Case management coordination, i.e., discharge planning, teaching complex medical care scenarios to patients and families. Strong verbal and written communication skills; the ability to engage in meaningful and solution-focused discussions with stakeholders and partners is imperative. Robust negotiation and mitigation skills; ability to remain professional and composed during times of adversity.
Preferred Qualifications
Home care experience, knowledge of transitional planning. Indigenous programming or health care certificates. Broad knowledge of Indigenous history within the healthcare system and the associated impacts on overall health outcomes. Fluency in Indigenous language is an asset.
