Your Opportunity
The Physiotherapist (PT)in the Stroke Early Supported Discharge (SESD) Program provides specialized assessment, treatment, education, and rehabilitation services to individuals transitioning from hospital to home following a stroke. The SESD program delivers a flexible hybrid mode of care, combining in-person and virtual services to support clients throughout their recovery journey. As part of an interdisciplinary team, the successful PT will promote recovery, functional independence, mobility, safety, and quality of life through evidence-informed assessment and intervention.
The role focuses on collaborative goal setting, patient and family education and support, community reintegration, and coordination of care across the continuum of stroke services. The SESD team is based at the Aberhart Centre on the University of Alberta Hospital campus. Centrally located in Edmonton, the site offers easy access to the University of Alberta, the vibrant Old Strathcona district, and the city's scenic river valley.

Description
As the Physiotherapist I (PT), you will provide therapeutic interventions to enable clients to achieve optimum physical function and independence in development, disease, disability, chronic conditions, illness and functional decline. You will provide intervention (i.e. prevention, triage, screening, assessment, diagnosis, goal setting, treatment, management, education, consultation, fostering resourcefulness, case management,
referral and transition support to individuals, groups, populations and communities. You will include clients and families as integral members of the health care team, encouraging their active participation in all aspects of care.
The PT collaborates with patient(s) and families in determining, implementing and evaluating appropriate rehabilitation strategies. You will participate in a variety of interprofessional teams to build awareness, enrich supports and engage in quality improvement; partnering with healthcare providers, patients, families and other stakeholders to advance services for their clients. You will contribute to quality improvement and their professional community; participates in peer mentoring; and supervises students.
You may provide clinical supervision to Therapy Assistants, Therapy Aides and Provisional Physiotherapists.

Required Qualifications
Bachelor's degree in Physiotherapy from an accredited university program. Current registration with the College of Physiotherapists of Alberta.
Additional Required Qualifications
Basic Life Support – Health Care Provider (BLS-HCP) from the Heart and Stroke Foundation.
Preferred Qualifications
As required.
