Overview

We are seeking a dedicated and compassionate Registered Practical Nurse (Part-time) to join our healthcare team. Duties

- Deliver comprehensive patient care by assessing health conditions, developing care plans, and implementing appropriate interventions.
- Administer medications and treatments accurately following established protocols and medical orders.
- Conduct clinical procedures such as phlebotomy (blood draws), wound care, and pain management.
- Monitor patient progress using medical terminology knowledge and document findings meticulously in electronic health records like Epic and Cerner systems.
- Collaborate with interdisciplinary teams to coordinate case management, discharge planning, and specialized care such as dementia, hospice, or assisted living services.
- Provide patient education on health management, medication use,



and lifestyle modifications tailored to individual needs.
- Assist with laboratory procedures and maintain infection control standards to ensure a safe environment for patients and staff.

Experience

- Proven experience working in nursing homes, hospitals, or acute care settings with a strong focus on patient-centered care.
- Demonstrated ability to handle diverse populations including pediatrics, seniors, and individuals requiring hospice or dementia care.
- Familiarity with home care environments and assisted living facilities is highly desirable.
- Knowledge of medical terminology, medication administration protocols, and laboratory procedures such as phlebotomy.
- Experience using electronic health record systems like Epic or Cerner for documentation and case management purposes.
- Background in pain management, hospice support, or senior care enhances your ability to deliver specialized services effectively.
- Strong communication skills combined with medical office experience are essential for collaborating with healthcare teams and educating patients effectively.
