Key Responsibilities:



• Conduct psychosocial assessments and establish intervention plans • Collaborate with interdisciplinary team members for care integration • Document patient interventions following OCSWSSW guidelines • Identify and intervene in high-risk social situations • Facilitate therapeutic counselling and case management for patients

Requirements: • Bachelor’s in Social Work required; Master’s preferred • 1 year of experience in Diabetes and Bariatric care • Registration with Ontario College of Social Workers mandatory • Familiarity with DSM and psychotherapeutic modalities • Commitment to patient-centered care in diverse populations
