As an LPN, you will report to the Resident Care Manager and be instrumental in delivering personalized care to residents. Your responsibilities encompass engaging the nursing team, collaborating with various professionals, and ensuring the highest standards of care based on Covenant Care's values. Additionally, you'll provide essential support to both residents and their families during sensitive end-of-life circumstances.

Key Responsibilities:
• Deliver personalized daily care to residents
• Collaborate with a multidisciplinary team




• Represent Covenant’s mission and values consistently
• Assist in end-of-life care planning and support
• Continuously assess and adapt care plans

Requirements:
• Graduate of an approved Practical Nurse Program
• Current CLPNA registration required
• CPR/First Aid Certification necessary
• Strong abilities in assessment and patient relations
• Experience in continuing care is preferred
