As part of VHA Home HealthCare, RNs and RPNs are invited to join our Palliative Care program. You will coordinate patient care, collaborating with community healthcare teams, and ensuring compliance with provincial nursing standards. Candidates must have a valid Ontario driver's license and reliable transportation for client visits.

Key Responsibilities:
• Develop and implement client care plans
• Monitor patient conditions and revise care strategies
• Uphold high standards in patient documentation
• Communicate effectively with clients and families
• Participate in team meetings and on-call schedules

Requirements:
• Active registration with the College of Nurses of Ontario
• RPN diploma or RN degree required
• Valid CPR or First Aid certification
• Experience in acute care is an asset
• Solid critical thinking and communication skills
