Join Ontario Shores Centre for Mental Health Sciences as a Registered Practical Nurse in the Hospital to Home Program. Focus on recovery-oriented care for patients with complex mental health conditions.

Reporting to the Clinical Manager for Complex Services, you will provide expert support to clients and families, ensuring effective transitions from hospital care to home. This position is essential for fostering relationships with multidisciplinary teams and community resources to enable client recovery.

Key Responsibilities:
• Implement recovery-focused care for patients with mental illness
• Work collaboratively with multidisciplinary teams and community partners
• Assist with patient discharges and recovery planning




• Ensure adherence to clinical standards of practice
• Engage in ongoing reflective practice

Requirements:
• Current registration with the College of Nurses of Ontario
• CPR certification for Health Care Providers required
• 5 years of psychiatric experience preferred
• Strong problem-solving and critical thinking skills
• G class Driver’s License required
