This part-time independent contractor position offers 22.5 hours weekly, centered on assessing and coordinating care for residents during their move-ins and move-outs. You will also assist families in accessing necessary resources, ensuring residents have the support they need for a seamless transition.

Key Responsibilities:
• Evaluate and assist residents in transitions
• Document assessments and provide follow-up support
• Collaborate with interdisciplinary teams
• Facilitate Memorial Services with Recreation Department

Requirements:
• Bachelor’s degree in Social Work
