Health Sciences North is looking for an Extended Length of Stay Risk Assessor to enhance their Hospital to Home program. This full time role involves conducting patient risk assessments and facilitating care transitions.
As a vital member of the Access & Flow team, you will assess patients for eligibility in the Hospital to Home initiative, ensuring streamlined pathways and effective discharge planning. Your collaboration with both internal teams and external stakeholders will be key to supporting improved patient experiences during their transitions.
Key Responsibilities:
• Engage patients in discussions about the Home First approach
• Evaluate risk using assessment tools like InterRAI
• Partner with community services for effective patient onboarding
• Provide documentation for assessments and recommendations
• Attend and participate in daily care meetings for patient status updates
Requirements:
• Bachelor’s Degree in a healthcare-related field
• At least two years of healthcare experience
• Experience with risk assessment tools preferred
• Knowledge of elderly care workflows
• Training in Lean or similar methodologies
