At AMANI: Mental Health & Substance Use Services for Black Youth, we are empowering youth and families through dedicated healthcare. As a full-time Nurse Practitioner, you will report to the Senior Manager of Child, Youth and Family Services, contributing to a racially informed care model. Engage in interprofessional collaboration and facilitate group sessions, while ensuring continuity of care through comprehensive assessments.
Key Responsibilities:
• Evaluate and monitor patient health using best practices
• Support interprofessional team consultations and resource management
• Lead and coordinate educational group sessions
• Execute crisis management and risk assessments
• Document patient progress and treatment plans
Requirements:
• Registered Nurse in Extended Class (RN(EC)) with Ontario
• Proficiency in mental health care practices
• Exceptional communication and leadership skills
• Familiarity with the Mental Health Act
• Willingness to work evening hours as necessary
