Bridges CHC is hiring two RPNs who are passionate about community health. Successful candidates will hold a diploma in Practical Nursing, be registered with the College of Nurses of Ontario, and have at least three years of experience in a related workplace. Strong communication and time management skills are essential to excel in this interdisciplinary role.

Key Responsibilities:
• Provide triage and appointment organization for clients
• Document and communicate client results and recommendations
• Coordinate intake processes for new primary care clients




• Ensure timely EMR documentation as per standards
• Administer injections and conduct wound care tasks

Requirements:
• Current RPN registration with the College of Nurses of Ontario
• Minimum three years' experience in community nursing
• Knowledge of electronic medical records systems
• Strong interpersonal and communication abilities
• Reliable vehicle and valid Ontario driver's license
