You will engage in cooperation with clients and multidisciplinary teams to devise personalized wellness plans that cater to diverse needs. Key Responsibilities:
• Develop tailored wellness plans in collaboration with clients
• Implement assessments and monitor client care

Requirements:
• Minimum requirements include a degree in Social Work or Psychology
