In this role, you will assess and care for patients, develop care plans in collaboration with families, and ensure a smooth discharge process. You will work closely within an interprofessional team to provide comprehensive care ensuring patient dignity and autonomy. Key Responsibilities:
• Collect and assess patient information for care planning
• Collaborate on care plans with families and health professionals




• Educate patients and families on health and wellness
• Maintain thorough documentation of patient care activities
• Engage in research and continuous learning initiatives

Requirements:
• Approved School of Nursing graduation
• Registered Nurse status with BCCNM
• Commitment to specialty nursing course completion
• Current Basic Cardiac Life Support (BCLS) certification
• Excellent communication and collaboration skills
