Take on a leadership role as a Social Worker with Park Place Seniors Living. This temporary part-time position provides essential support for current Residents, working Monday through Wednesday.

As the Social Worker, you will guide the interdisciplinary team in the critical Move-In processes for Residents at Park Place Home. Your role includes developing individualized support programs that meet the diverse needs of Residents and their families. You'll also take charge of ethical program initiatives, ensuring a culture of safety and well-being.

Key Responsibilities:
• Oversee Resident Move-In and Move-Out evaluations
• Provide transitional support and resources to families
• Participate in interdisciplinary care conferences




• Ensure timely documentation of social work activities
• Lead quality improvement initiatives at the home

Requirements:
• Degree in Social Work from an accredited school
• Relevant social work experience, particularly with the elderly
• Excellent communication and decision-making skills
• Must be organized and flexible
• Proven problem-solving abilities in teamwork
