WHAT THE ROLE PROVIDES YOU:

KEY RESPONSIBILITIES

Responsible for oversight of ongoing placement and care of Clientschoosing an integrated living arrangement

Collaborate with HR on recruitment, selection and orientation of newIntegrated Living Practitioners (ILPs) and ONR applicants

Facilitate and monitor contractual agreement between individualClients and the Integrated Living Practitioner/Guardian/Families

Initiate monthly conversations with new clients, networks, and newILPs who have been with the agency for less than one year

Consult with ILPs and ONR providers during crisis situations that impactthemselves, the Client and/or their family

Liaise and maintain contact with families and guardians at least quarterly;discuss pertinent matters, required documentation, service and any concerns

Responsible for the application process and associated meetings withprospective ILS clients





Monitor Client ILP and ONR assigned caseload files regularly forcompliance with policies and Creating Excellence Together (CET) standards;ensure audits, visits, and reviews are completed in a timely manner

Facilitate the planning, development and implementation of clientgoals; coordinating and participating in individual planning meetings;coordinate communications with other service agencies to ensure individual outcomesare met; monitors, documents and reports progress toward meeting outcomes;ensures required supports are in place to meet outcomes

Alerts the Manager or designate, to any serious concerns

Ensure appropriate and timely incident reporting occurs and ensure recommendationsare implemented

Provide assistance to clients with medical, psychological, andphysical appointments; providing medical history support for independentclients

Support clients, parents and guardians by researching information onavailable services from agencies such as OPG, AISH, and Calgary Housing

Educate ILPs, ONR providers, networks and clients regarding DDRC’sPolicies and CET standards

Monitor training of ILP and ONR providers

Report to the Manager of HBS any outstanding training or individualswho do not respond to requests to participate in mandatory training

Ensure Client files and DDRC records are created and maintained inaccordance with DDRC protocols, provincial requirements, funding, andaccreditation standards, policies and procedures

Prepare and maintain program documentation according to establishedstandards and requirements; includes but not limited to:

Caseload summariesfor both Clients and ILPs

Individual FundingProposals and reviews

Actively participate as a member of DDRC Review Team as directed by the Manager,HBS

Prepare required and assigned necessary correspondence for Manager,HBS review

Assist in the development of proposals, standards and methods toimprove the quality of services for Clients and/or ILPs in the ILS program





Perform all other duties assigned by the Manager, HBS or designate

Takes accountability to ensure all mandatory certifications andtraining are kept current

Attends all mandatory agency required training sessions

Completes all required Performance Management documentation; attendscheduled reviews with Manager

Represents the ILS team at public meetings as required and serve onrelated committees in the community

WHAT YOU BRING TO THE ROLE:

Post-Secondary diploma in Social Work, Human Services, or an acceptablecombination of education and experience

Minimum of three (3) years leadership experience within a fast-pacedenvironment

Experience with building strong positive relationships

Experience working with adults with developmental disabilities,including PBS supports

Knowledge of community resources for mental health and addictionissues

Experience in crisis management, conflict resolution and mediation

Robust critical thinking, excellent organization and administrativeskills

Strong Leadership, Relationship Building and Conflict ResolutionSkills

Customer Service and Time Management are a must

Demonstrated proficiency in MS Office, with particular emphasis on MicrosoftExcel, Word and PowerPoint

Knowledge of and understanding of applicable federal and provinciallegislation as well as professional standards

Creating Excellence Together (CET) Accreditation experience within thedisability sector, an asset

Travel to all quadrants and communities within Calgary City Limitsyear round

Provide proof of business commercial automobile liability coveragewith a minimum of 2 million liability
