Mental Health Case Worker

TNO - The Neighbourhood Organization (TNO) is a well-established community-based social and health service agency providing a wide range of programs and services supporting low-income and newcomer communities across Ontario and sometimes across different provinces through online services and referrals. We are seeking a Mental Health Case Worker to join our team for a full-time permanent role.

The Mental Health Case Worker will engage clients throughout the community, including in their homes by building trust through informal connections and by meeting felt needs that can help reduce stressors in the client’s life. The ideal candidate should possess a Bachelor of Social Work (BSW) or equivalent education and related experience in a relevant field. Registration with the Ontario College of Social Workers and Social Service Workers or an equivalent regulatory body is required. They must have 2–3 years of related experience in crisis and trauma support, conflict mediation, and providing frontline services and outreach to seniors, newcomers, and diverse communities. Strong assessment, analytical, and decision-making skills are essential, along with experience supporting clients with mental health and substance use issues using recovery- and harm-reduction approaches. Excellent organizational, team-building, communication, and interpersonal skills are a must, with the ability to work both independently and collaboratively while responding to changing community needs. Proficiency in Microsoft 365 applications, database software, reporting tools, and survey tools is an asset. Proficiency in multiple languages is also an asset.

DUTIES & RESPONSIBILITIES

- Complete assessments and histories of clients as required and assess additional support required based on specific areas of need (e.g., mental health, physical health, etc.)
- Cultivate linkages with the community members and influencers and the people affected by addictions and build rapport and trust; collaborate with stakeholders in the community to identify and respond to needs
- Collaborate with partners and other external service partners to advocate for client needs,



make referrals and follow up to ensure connection shave taken place
- Provide coordination of care promote and support clients’ participation and inclusion in the community
- Connect clients and participants of the program to case management, one on one mental health supports and rehab services
- Complete home visits as required; support clients actively for the pre and post- treatment period in the neighborhoods
- Provide short term supportive and crisis counselling to clients with mental health, emotional health and substance abuse issues
- Accompany clients to appointments as needed
- Participate in team meetings and center-wide initiatives
- Perform administrative tasks, maintain accurate records and write monthly reports
- Participate in the development and evaluation of TNO programs and service
- Participate in the development, implementation and evaluation of programs
- Maintain up-to-date work plans.
- Use evaluation methods and reporting tools to gather and maintain statistical information on activities and programs
- Work with students, volunteers and/or community members
- Share programming evaluations and reflections with Community Engagement & Wellness and other relevant teams
- Input data into appropriate data base as specified by the agency
- Maintains required program records and submit required weekly and monthly reports and as requested by the management of the agency
- Develop monthly work plans and activities
- Ensure all records are kept confidential and secure
- Explore and expand community development approaches
- Develop individualized outreach plans for each assigned service locations in collaboration with other TNO programs or external service providers
- Act as a resource to the community and TNO staff by establishing links with community groups, agencies and Community Ambassadors




- Actively conduct outreach to the community by organizing/attending community events, promoting TNO programs, updating internal information boards, and working on TNO’s promotional materials for Health services
- Collaborate with stakeholders in the community to identify and respond to needs
- Participates in networks of neighborhood-based service providers
- Participates in and supports local community events.

Screening – As a condition of employment, the successful applicant must complete a Vulnerable Sector Screening.

Job Type: Full-Time permanent position.

Pay rate: $28.00 - $30.00 per hour.

Schedule: 35 hours per week, scheduled between 8:30 AM and 8:00 PM. Some evening and weekend work may be required based on program or departmental needs.
