Job Summary

Duties
- Deliver comprehensive patient care by assessing needs, developing care plans, and implementing treatments in accordance with medical directives.
- Administer medications and injections accurately, following prescribed protocols and safety guidelines.
- Utilize electronic health record systems such as Epic and Cerner to document patient information, treatment plans, and progress notes efficiently.
- Perform phlebotomy procedures to collect blood samples for laboratory analysis while maintaining strict safety standards.
- Monitor vital signs, manage pain levels, and provide comfort measures to enhance patient well-being.
- Collaborate with physicians, case managers, and multidisciplinary teams to coordinate care for patients with complex needs including pediatrics, senior care, dementia, or post-acute recovery.
- Conduct patient education on medication administration, disease management, and health maintenance strategies.
- Support laboratory testing procedures and interpret basic medical terminology to ensure accurate communication of results.
- Assist with medical office tasks when needed, including scheduling and patient intake processes in various healthcare settings.
- Engage in home care visits or assisted living environments to deliver personalized nursing services tailored to individual needs.
Qualifications
- Valid RN license in valuable standing with relevant state licensing boards.
- Proven experience in hospital settings with acute care exposure; experience in pediatrics, senior care or dementia care is highly desirable.
- Strong knowledge of Epic and Cerner electronic health record systems; familiarity with medical terminology is essential.
- Experience working with children or in home care environments is a plus.
- Skilled in medication administration, pain management techniques, phlebotomy procedures, and laboratory processes.
- Excellent communication skills with the ability to educate patients effectively on their health conditions.
- Prior nursing home or assisted living experience preferred; familiarity with medical office operations is advantageous.
- Ability to work collaboratively within multidisciplinary teams across various healthcare settings including hospitals and community clinics.
- Demonstrated competence in case management principles and patient-centered care approaches.
