Access Healthcare Services in Ottawa seeks a passionate Client Care Supervisor to enhance home and community healthcare. Deliver high-quality assessments, training, and supervision in this hybrid role. As a full time RPN, you will provide leadership in client-centered care while coordinating with Personal Support Workers.

Your responsibilities include developing care plans, monitoring service delivery, and collaborating with interdisciplinary teams. Strong clinical judgment and exceptional customer service will be key to your success. Key Responsibilities:

- Conduct comprehensive assessments for effective care
- Create and implement client-centered care plans
- Mentor Personal Support Workers and Home Support Workers
- Ensure quality assurance through regular service monitoring
- Participate in quality improvement initiatives Requirements:
- Graduation from an accredited Licensed Practical Nursing program
- Active registration with the College of Nurses of Ontario
- At least 12 months’ experience in community health roles
- Proven ability to handle HR responsibilities
- Evidence of current CPR certification Support optimal health and service delivery for clients across Ottawa with Access Healthcare Services.
