The Aspen PCN is seeking a Social Worker (0.5 FTE, Temporary) based in Athabasca, AB. You will perform case management, connect clients with housing, DIA/EI programs, and seniors services, and collaborate with physicians and community teams to support patient self-management.
Responsibilities include navigating funding opportunities, maintaining EMR data, and delivering patient education. Must hold a Bachelor’s in Social Work and be registered in Alberta; valid driver’s license required.
