House is seeking a dedicated Shelter Case Manager to lead comprehensive case management for at-risk youth. In this permanent position, you'll offer intensive support and develop individualized plans addressing housing, healthcare, and resilience needs. Your leadership within the Case Management Team will ensure high-quality service delivery while navigating youth through complex challenges. Key Responsibilities:

- Provide intensive case management using a client-centred approach
- Develop therapeutic relationships to foster trust and engagement

- Facilitate access to community resources and supportive services
- Maintain accurate documentation and case notes for effective tracking Requirements:
- BSW or MSW degree with 5 years relevant experience
- Registration with the College of Social Workers and Social Service Workers
