Department/Unit/Program Description The RMH Health First - Diabetes Education and Management Program provides assessments, follow-up and ongoing support for adults with pre-diabetes, gestational diabetes, Type 2 diabetes and Type 1 diabetes, including individuals using insulin pumps. Our team of experienced and passionate Certified Diabetes Educators are committed to empowering individuals to self-manage diabetes and obtain optimal health. Providing client-centered care is a priority as the team works with each client, their primary care, specialists and other community partners to develop personalized care plans to meet each individual’s unique needs and lifestyle.
Position Summary The Diabetes Educators are key players in the initiation of insulin pumps for new starts, and the assessment of Type 1,
Type 2, and Gestational diabetes patients.
The Diabetes Clinic is currently seeking a Permanent Full-Time Diabetes Nurse Educator (RN) to join their healthcare team.
Effective Date: October 13, 2026
Essential Qualifications
- Current Certificate of Registration with the College of Nurses of Ontario in good standing as a RN
- Successful completion of certificate in diabetes education from a recognized program
- Certified Diabetes Educator
- Minimum three years current experience as a Diabetes Educator within the last five years
- Demonstrated performance within the current evidence-based guidelines for managing diabetes and the Canadian Diabetes Standards
- Strong understanding of the impact of chronic diabetes on the patient and family
- Solid understanding of Delegated Medical Acts and the RN's responsibilities when performing these acts
- Participation in the College of Nurse's Quality Assurance Program
- Recent experience, skill and knowledge in adult teaching theories
- Demonstrated expert knowledge base of diabetes and the methods used to support patients with the diagnosis
- Demonstrated ability to communicate (highly developed written and verbal communication)
and work effectively with the multi-disciplinary team and the members of the community
- Demonstrated high level communication, organization and time management skills
- Demonstrated knowledge of Word, Excel, and Power Point
- Expertise in developing and maintaining excellent relationships with a broad range of individuals and partner organizations
- Demonstrated ability to work independently and efficiently
- Demonstrated ability to meet the attendance standard set by the Ross Memorial Attendance Awareness Policy
- Must have a valid drivers license and vehicle
- Demonstrated commitment to professional development and continuing education
- Must be able to fulfill physical requirements of the job, as per current Physical Demands Analysis
- Must meet expectations for appropriate, respectful and professional conduct, as set out in the RMH Code of Conduct
Preferred Qualifications
- Experience with craving change
- BScN
- Strong understanding of equipment and technology used in diabetes management
- Competent in the management of cardiac and pulmonary chronic diseases
Hours of Work
Must be available to work eight (8) hour shifts Monday to Friday. Shifts and hours may change according to departmental requirements and as per the ONA Collective Agreement.
