Health Promoter

Position Summary The Health Promoter works with Prime Care’s interprofessional team and community partners to design, coordinate, promote and evaluate a consistent portfolio of evidence‑informed programs. The role ensures programming aligns with patient and community needs, Prime Care’s strategic direction and Ontario Health priorities. This position provides structure for program planning and delivery; clinical staff retain responsibility for clinical content and standards.

Duties & Responsibilities

● Maintain a current program inventory and annual/quarterly program plan.

● Design and refine programs with interprofessional staff using evidence, adult‑learning and equity principles.

● Recommend program continuation or changes.

● Maintain a reliable program calendar.

● Coordinate scheduling, registration, space, materials and virtual delivery.

● Create reusable program packages and resources.

● Monitor program delivery and flag capacity/resource issues.

● Provide referral guides and updates for physicians; engage them to understand referral needs.

● Promote programs through community partners.

● Identify and address language, cultural and access barriers.

● Build community relationships and use feedback to improve reach.

● Track basic program data and prepare quarterly dashboards.

● Support required organizational/provincial reporting.

● Ensure privacy and confidentiality.

● Participate in staff meetings and committees.

● Contribute to an inclusive, respectful workplace.

● Support occasional evening/community programming.

● Perform other related duties as required.





Role Boundaries The role does not provide clinical care, carry a caseload, act as general admin support or serve as the sole communications resource. The Executive Director approves the annual program portfolio and major changes

Qualifications & Requirements

● Bachelor’s degree in public health, health promotion, adult education, community health or related field, or equivalent experience.

● 3–5 years designing, coordinating and evaluating health or community programs.

● Experience working with interdisciplinary teams and community partners.

● Experience developing workplans, workflows, educational materials and evaluation tools.

● Strong facilitation, relationship‑building, communication and organizational skills.

● Competence with standard office applications; EMR experience an asset.

Preferred Assets

● Experience in primary care, Family Health Teams or community health.

● Training/experience in quality improvement or project management.

● Multilingual

Additional Requirements

● Ability to travel between Prime Care and community locations.

● Ability to support occasional evening programming.

Employment Type: Full‑time (1.0 FTE)

Reports To: Executive Director

Location: Prime Care sites and community locations in Milton

Salary: $77,673 annually
