• Deliver individualized chronic disease care education • Utilize adult learning principles for program development • Provide assessment and intervention in chronic disease management • Record and report client care accurately using EMR • Adapt strategies for diverse client backgrounds and needs

Requirements: • Bachelor of Nursing with current RN registration • Preferred certification as a Chronic Disease Educator • Two years of chronic disease nursing experience • Proficiency in Microsoft Office and communication tools • Basic Life Support certification valid at hire
