Role: Medical Social Worker for dialysis patients
Responsibilities
- Assess psychosocial needs of patients and families
- Provide counseling and emotional support
- Coordinate community resources to assist with chronic kidney disease management
- Collaborate with nephrologists, nurses, and dietitians to support patient care
- Support treatment adherence and care transitions
- Advocate for patient well-being
Qualifications & Skills
- Strong interpersonal and communication skills
- Ability to assess psychosocial needs and provide counseling
- Team-oriented mindset with collaborative skills
- Knowledge of chronic kidney disease and dialysis care preferred
