Enhance community well-being as a Community Health Nurse with the Northwest Territories Health and Social Services Authority. This role involves patient-centered care delivery and a focus on health promotion initiatives. As part of the Integrated Care Team, you’ll work collaboratively to assess and manage health conditions while providing education and support to patients and families.
Your efforts will foster healthier communities through coordinated healthcare services and advocacy for equitable access. Key Responsibilities:
- Deliver primary and community health nursing services
- Implement health promotion and disease prevention strategies
- Collaborate with interdisciplinary teams for comprehensive care
- Monitor health screenings and manage acute/chronic conditions
- Document clinical activities accurately and promote quality improvement Requirements:
- Nursing Degree and two years of recent nursing experience
- Experience in ER or Community Health settings
- Valid registration with the College of Nurses of NWT and Nunavut Support health advancement within communities through dedicated nursing care and teamwork.
