Duties
- Virtually: Manage prescriptions refill and reports review.
- Virtually: Address any concerns appropriate for virtual care.
- When in-person: Conduct thorough patient assessments, including history-taking and physical examinations for pediatric, adult, and senior patients.
- Develop and implement personalized treatment plans, including medication management and health education.
- Administer medications, perform procedures, and monitor patient responses using advanced medical techniques.
- Collaborate with physicians, nurses, case managers, and other healthcare professionals to coordinate care across various settings such as clinics, nursing homes, assisted living facilities, and home care environments.
- Manage chronic conditions like dementia and pain management issues while providing acute care when necessary.
- Utilize electronic health record systems: Oscar Pro to document patient encounters accurately and efficiently.
- Assist with interpretation of reports and laboratory testing.
Requirements
- Valid Nurse Practitioner license with current certification in your practicing state.
- Experience with electronic health records systems such as Oscar Pro is preferred.
- Demonstrated ability to deliver high-quality patient care in fast-paced settings including acute and chronic care environments.
- Excellent communication skills to effectively educate patients and collaborate with healthcare teams.
- Additional experience in pediatrics, dementia care, laboratory procedures like phlebotomy, or home care services will be considered advantageous.
Expectations:
- Working virtually 5-6 hours per week to review reports and refill prescriptions while ensuring tests and screening are up to date.
- Every other week: Working 3-4 hours in person for acute patient care.
Pay: $55.00-$58.00 per hour
Expected hours: 5.0 – 10.0 per week
