Job Summary

We are a rehabilitation clinic supporting patients recovering from motor vehicle accidents (MVAs). We are seeking a compassionate and reliable PSW to provide in-home support to our MVA patients throughout their recovery.

The PSW will be responsible for completing scheduled home visits and assisting patients with their daily living needs while promoting safety, independence, and overall well-being.

Responsibilities

- Provide in-home personal support to MVA patients as outlined in their care plan
- Assist with activities of daily living, including personal care, mobility, and household-related tasks as required
- Monitor and report changes in the patient's condition or needs
- Maintain accurate documentation following each home visit
- Communicate professionally with the clinic and rehabilitation team
- Follow individualized care plans and safety protocols




- Maintain patient confidentiality and professionalism at all times
- Travel to patients' homes within the assigned service area

Qualifications
- Valid PSW certificate from a recognized institution
- Previous experience providing in-home care is an asset
- Experience working with MVA or rehabilitation patients is an asset
- Valid CPR/First Aid certification
- Reliable transportation and ability to travel to patient homes
- Solid communication and interpersonal skills
- Compassionate, dependable, and professional approach
- Ability to work independently while following a care plan
- Vulnerable Sector Check may be required
